Your tattoo will be carried out professionally with sterile equipment, in a registered and hygeinic premises. You will be given aftercare instructions on how to properly care for your tattoo to promote good healing. By signing below you are indicating that you acknowledge and understand the following:
We accept no liability for poor healing due to lack of aftercare on your part.
We accept no liability for the design once you have approved it and the tattoo is complete.
You are aware that you should consult your GP as soon as possibleshould any signs of infection occur.
Our current price for re-touches is £50+
For ongoing work you will not need to fill this form out again but it is your responsibility to inform us should any of your information change.
